Registered Nurse, Sub Acute Skilled Nursing Unit - Part Time

OVERVIEW

Provide advanced, patient‑centered nursing care while working closely with physicians and the healthcare team to assess, treat, educate, and support patients and families throughout their care journey.

This role also serves as a clinical leader by mentoring team members, improving patient care practices, supporting quality initiatives, and helping create a positive, collaborative healthcare environment.

SCHEDULE:

16‑hours per week (two, 8‑hour shifts)

• Utilizes the nursing process and collaborates with interdisciplinary team members in caring for patients.
• Administers prescribed medications and treatments in accordance with approved nursing techniques, policies and procedures, standards of care, and State laws/practice act.
• Performs assessments, planning, intervention and evaluation of care.
• Documents according to standards of care.
• Develops, initiates, and executes a patient plan of care according to the individualized needs of the patient, as prescribed by Physician and/or clinic policy including patient and family instruction.
• Participates in department or unit performance improvement activities, may include shared governance.
• Utilizes clinical expertise and guidance to other units and/or departments in support of the patient’s longitudinal plan of care.
• Utilizes critical thinking regarding patient safety and patient experience and appropriately escalates to the appropriate chain of command.
• Participates in the professional nursing continuum process, unit practice activities and decisions and peer review process.
• Offers ideas and/or solutions to practice issues.
• Uses innovative methods and intuitive skills to develop and implement plans of care.
• Provides innovative ways to educate patient‑families.
• Initiates interdisciplinary collaboration to meet the patient/family educational needs.
• Actively participates in mentoring and precepting new partners to the organization.
• Positively influences the professional and organizational environments and shared governance.
• Engages in personal development and supports system‑entity opportunities for continuous learning and growth, including developing or revising evidence‑based practice guidelines.
• Participates in a minimum of 1 departmental or unit performance improvement activity, may include shared governance.

Graduate of an accredited nursing program. ADN required upon hire, BSN preferred.

• Licensed as an RN in state of practice
• Approved CPR/BLS certification required within 90 days
• Other certification(s) per unit requirements (e.g., ACLS, PALS, PEARS, NRP)

• Passing the Driver’s License Check and/or Credit Check (for those positions requiring).
• Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check.
• Must be able to follow written/oral instructions.

The noise level in the work environment is usually moderate. Occupational Exposure:
Category A – All partners in the specified job classification have occupational exposure.

• RN Hospital, RN Home Health and Hospice, RN Oncology: regularly required to walk and talk or hear, frequently required to stand, use hands to finger, handle, or feel and reach, occasionally required to sit, kneel, crouch, or crawl.
