Patient Health Navigator
Community Health Partnership of Illinois is a non‑profit organization that provides primary health care to migrant farm workers and other medically underserved populations throughout northern and central Illinois, specializing in primary preventive medical, dental, and behavioral health services. Our focus is delivering quality, culturally responsive health care services to our patients, caregivers, and partners in the communities we serve.
The Patient Health Navigator builds effective working relationships with patients, helping secure medical appointments for primary and specialty care, provide education, and assist patients in navigating the complex health care system. PHNs collaborate with CHP’s multidisciplinary care teams, providers, and community partners to improve patient outcomes. PHNs conduct intake interviews, provide regular follow‑up care and documentation, and facilitate referrals to additional services as necessary.
Professional& Technical Knowledge
- Experience working in community work, such as advocacy or volunteering.
- Ability to see the inherent worth and dignity of each client.
- Experience assisting underserved community residents with community resources.
- Experience as a Certified Application Counselor and maintenance of certification.
- Effective communication with management and other departments.
- Strong understanding of business goals and standards for customer service.
- Experience working with non‑profit or social service organizations.
- Ability to effectively organize and manage multiple client caseloads and encourage coworkers to do the same.
