A leading healthcare institution in Illinois is seeking an Intake & Referral Nurse Liaison (RN) for its Home Hospice unit. The successful candidate will be responsible for facilitating the patient admission process, conducting assessments, and collaborating closely with patients and families to ensure quality hospice care. Applicants must have a valid RN license in Illinois and a BLS certification, alongside prior acute care experience.

This role is full-time, onsite, and based out of the Hyde Park campus with occasional home visits.
