MDS Coordinator
Join the Regal Care at Harwich Team!

We are seeking a dedicated and detail-oriented MDS Coordinator to join our team at Regal Care  a key member of our healthcare team, you will play a vital role in ensuring the highest quality of care for our patients and residents.

About the Role:

As an MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) assessment process, ensuring accurate and timely completion of assessments, and maintaining accurate and up-to‑date patient and resident records. You will work closely with our medical team, families, and other healthcare professionals to provide comprehensive care to our patients and residents.

Responsibilities:
  • Coordinate the MDS assessment process, including scheduling and facilitating assessments with interdisciplinary teams

  • Ensure accurate and timely completion of MDS assessments, RAPs, and other required documentation

  • Maintain accurate and up‑to‑date patient and resident records, including medical and social histories

  • Collaborate with medical teams, families, and other healthcare professionals to ensure comprehensive care

  • Develop and implement effective communication strategies to ensure seamless coordination of patient and resident care

  • Identify and address any discrepancies or inconsistencies in patient and resident records

  • Participate in quality improvement initiatives to improve patient and resident outcomes

Requirements:
  • High School Diploma or equivalent required

  • MDS Coordinator experience preferred

  • Strong organizational and analytical skills, with attention to detail and ability to work in a fast‑paced environment

  • Excellent communication and interpersonal skills, with ability to work effectively with diverse stakeholders

  • Ability to maintain confidentiality and handle sensitive information
