Suncrest Hospice is seeking a committed Registered Nurse to provide exceptional hospice care in a community setting. We aim to improve patient and family experiences by enhancing staff‑to‑patient ratios and ensuring high‑quality clinical visits.

• Provide care and services for a group of patients, visiting households, Skilled Nursing Facilities, and assisted living facilities.
• Contribute to interdisciplinary team with psych‑social assessments, interventions, and knowledge of community resources.
• Work in Case Management, Admissions, or Resource roles.
• Maintain current RN licensure (in good standing) in the state(s) of practice.
• Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1‑to‑1 level with support from care team.
• Operate independently, managing phone, email, meetings, and comprehensive documentation.
• Effectively manage time, schedule visits, attend required meetings, and triage urgent needs.
• Learn an electronic medical records system.
• Drive with reliable transportation, valid driver’s license, and auto insurance to travel across service area.
