Triage Visit Registered Nurse - Home Care
HoursPart‑time, Weekend Days – available every other weekend to take calls, triage patients, and make visits if needed. Work remotely and visit patients when applicable. General hours: 8:00 am – 5:00 pm Saturday and Sunday.
TrainingAvailability Monday – Friday, 8:00 am – 5:00 pm, to train 2 days per week for 2 to 6 weeks as required.
Responsibilities- Take calls from the On Call Service and address them based on call type (e.g., Intake/Referrals, Scheduling, Patient Concern).
- Provide telephone triage and conduct patient visits as appropriate.
- Complete accurate and timely documentation of patient/family services provided.
- Provide education/instructions to patient/family in accordance with the plan of care.
- Administer medications and treatments as prescribed by the physician and in accordance with nursing standards of practice.
- Hourly Pay Range: $38.46 – $46.86

- Graduate of an accredited school of nursing and licensed to practice as an RN in the state of MN. License active and in good standing.
- Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years.
- Able to travel within a geographic area serviced by the office. Valid driver’s license, reliable transportation, and proof of auto insurance required.
- Current CPR Certification required.
