Home Care Licensed Practical Nurse (LPN):
Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN will provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.
- Pay Range: $30.42 - $36.98/hour

Full-time. Monday – Friday with weekend and holiday rotation (approximately one weekend every 4–6 weeks and 2 holidays per year). Rotation depends on staffing levels.
TerritoryField work in patients’ homes in the southeast metro area; office in Roseville, MN. Must travel within the geographic area serviced by the office.
Qualifications- Current, active Licensed Practical Nurse (LPN) license in Minnesota.
- Preferred: 2+ years experience as an LPN.
- Valid driver’s license, reliable transportation, proof of auto insurance.
- CPR Certification.
- Participate in ongoing interdisciplinary assessment of patients.
- Provide services ordered by physicians as indicated in the plan of care.
- Recognize and report changes in patients’ conditions to the RN.
- Participate in development and evaluation of care plans in partnership with patients, representatives, and caregivers.
- Counsel and educate patients and families regarding healthcare needs.
- Complete timely and accurate clinical notes documenting patient progress.
