The Home Health RN Case Manager is responsible for planning, organizing, and directing Lifespark services while ensuring compliance with regulatory requirements and best practices. The professional nurse builds from the resources of the community and care team to plan and direct services to meet the needs of individuals and families within their homes and communities.

Position: RN Case Manager for Home Health

Salary Range: $88,000–$90,000 annually, depending on experience.

Service Area: Southeast Metro (coverage area:
Hastings, Afton, Farmington, Inver Grove Heights, Cottage Grove, South St. Paul, West St. Paul, St. Paul Park, Rosemount, Eagan, Woodbury & surrounding). Additional mileage reimbursement of 72.5 cents per mile.

Schedule: Monday – Friday 8:00 am‑5:00 pm (with rotation every 6th weekend and 2 holidays/year and on‑call as needed).

Evening RN on‑call shift PRN: Paid $125, with PPV for visits completed: SOC $145, ROC $115, NSG Eval, Recert, Oasis DC $85 and SN Visit $70.

Position Functions & Responsibilities:

  • Complete an initial client and family assessment to determine home care needs; provide a complete physical assessment and history.
  • Initiate and revise the plan of care as client status and needs change.
  • Use health assessment data to determine nursing diagnosis.
  • Develop a care plan with goals based on nursing diagnosis and incorporate therapeutic, preventive, and rehabilitative nursing actions; involve the client and family.
  • Administer medications and treatments as prescribed by the physician and provide direct client care as defined in the State Nurse Practice Act.
  • Counsel the client and family to meet nursing and related needs.
  • Provide health care instructions to the client as appropriate per assessment and plan of care.
  • Identify discharge planning needs and implement prior to client discharge.
  • Act as Case Manager when assigned by the Clinical Manager and coordinate client care for the assigned caseload.
  • Lead by example in establishing rapport with clients, clinicians, partners, and the community.
  • Ensure positive and professional communication within the team.
  • Resolve customer service concerns and escalate as necessary.
  • Prepare clinical notes and update the primary physician at least every 60 days.
  • Communicate with the physician regarding client needs and report any changes; obtain physician orders as required.
  • Coordinate the care plan with community health related persons.
  • Complete all documentation related to the care plan by 10 am the next day.
  • Understand and comply with all regulations governing registered nurse responsibilities.
  • Complete orientation and annual training requirements as assigned.
  • Participate in event management, including initiating reports, escalating events, and investigations.
  • Promote workplace safety through real‑time coaching and safety training participation.
  • Instruct, supervise, and evaluate home health aide care provided every two weeks.
  • Manage client supplies ensuring appropriate DME provider is utilized.
  • Schedule and keep member appointments.
  • Organize travel efficiently and document mileage accurately within EMR.
  • Perform other duties as assigned.

Home Health RN Case Manager

Qualifications:

  • 2 years of RN experience in health care (preferably home care or hospice).
  • Medicare Skilled Nursing experience and basic understanding of OASIS strongly preferred.
  • Experience in customer service or direct member contact preferred.
  • Experience supervising caregivers.
  • Knowledge of Medicare Home Health & Hospice regulations.
  • Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practice.
  • Graduate of an accredited school of nursing.
  • Must pass a motor vehicle records check per company policy.
