Home Health Care, Inc. is seeking a dedicated RN Case Manager to deliver one-to-one home visits across Woodbury, Hastings, St. Paul Park and nearby areas.
You’ll enjoy a full-time, Monday–Friday schedule with occasional weekends and on-call shifts, while benefiting from a supportive clinical team. You will complete OASIS assessments, create individualized plans of care, provide skilled nursing visits and education, coordinate with physicians, and document accurately.
