Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses

What You'll Do Manage a home health caseload throughout Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall. Complete OASIS assessments and individualized plans of care. Provide skilled nursing visits, assessments, and patient education. Coordinate care with physicians and the interdisciplinary team. Help patients remain safe and independent in their homes. Maintain accurate and timely clinical documentation.

What We're Looking For Active Minnesota Registered Nurse (RN) license. 3+ years of RN experience (home health experience preferred). Strong clinical judgment and ability to work independently. Excellent organization and communication skills. Reliable transportation for travel throughout the territory.
