Responsibilities- Serve as caregiver to the resident during the scheduled work period in long-term care, delivering resident-centered nursing care and daily living assistance under the supervision of a registered nurse (RN).
- Provide age-appropriate care related to the physical and psychological needs of residents as per care plans.
- Follow infection prevention and control policies and use Personal Protective Equipment (PPE); be accountable to the nursing team.
- Assist with activities of daily living, including bathing, toileting, grooming, dressing/undressing, feeding, and documenting vital signs.
- Support transfer, repositioning, walking, and range-of-motion and passive exercises using correct techniques and equipment.
- Participate in meal and dining support to create a positive dining experience; assist with meals and snacks as appropriate.
- Assist providers and nursing staff with procedures as needed; document resident interactions and care outcomes.
- Monitor and communicate changes in resident condition to the charge nurse; collect non-invasive specimens and vital signs.
- Provide housekeeping and laundry services to maintain clean and functional living areas.
- Support wellness and recreational programs and encourage resident participation; participate in departmental or organizational committees as needed.
- High school diploma or GED preferred, but not required.
- Minimum age 16. Prior clinical or nursing assistant experience in long-term care preferred.
- Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state-certified CNA within four months of date of hire by enrolling in a training program or pursuing certification through Endorsement. CNA training and certification required and provided by the facility.
- Completion of yearly department-specific competency testing and maintaining continuing education credits (minimum 12 hours annually).
- Maintain all department-specific required certifications; if applicable, be certified and registered with the Unlicensed Assistive Person (UAP) Registry.
- Depending on facility and responsibilities, may classify as Sanford Category II Driver requiring a motor vehicle report and a valid driver’s license; must be medically qualified to perform essential driving functions as per the Sanford Driving Policy.
