Community Alliance, a pioneer in community mental health care in Nebraska for four decades is seeking a Full-Time Registered Nurse or LPN with clinic experience to work in our integrated health care setting providing services to behavioral health and primary care patients. This position requires a RN or LPN license. BLS certification is required.

Under the direction and supervision of the Clinic Manager
- Integrated Health Care Services, the Registered Nurse (RN) or LPN is responsible for the delivery of primary and behavioral health care and administrative responsibilities that places an emphasis on quality of care and customer service within an integrated care setting. The RN or LPN will work collaboratively with all clinic services staff and other Community Alliance program staff in support of direct patient care.

An individual in this role utilizes his/her professional knowledge, skills, and judgment in the conduct of his/her duties and contributes to the maintenance of a safe, supportive, rehabilitative, engaging, and welcoming environment for persons served and visitors to the organization.

Duties Include:

  • Prepares patient for provider exam by reviewing medical records, weighing patients, and CO2 level, checking vital signs, performing medication reconciliation, and obtaining information relevant to the patient’s holistic care plan.
  • Performs medication reviews and works with patients toward medication adherence.
  • Handles prescription renewals and inquiries to facilitate quality patient care.
  • Performs telephone triage to determine patient needs by recording and reporting patient condition to provider and advising patient based on provider instructions.
  • Returns patient phone messages from the Nurse Line according to protocol and documents disposition in the EHR system.
  • Anticipates, recognizes, prevents, and resolves problems before they occur so the issues can be addressed before they escape into bigger problems in the future.
  • Applies critical thinking skills and problem-solving techniques throughout the day and remains flexible and adaptable based on patient needs.
  • Assists and/or performs phlebotomy with proper labeling and handling of specimens, accurately completes requisitions, and collects specimens according to standard procedures/protocols and performing and documenting quality control activities.
  • Administers medications and vaccines as directed by providers.
  • Completes EKGs.
  • Assists providers with routine and complex procedures.
  • Records patient information, encounters, and charges utilizing electronic health record and other technology.
  • Collaborates and coordinates with other team members and other organizations regarding the patient’s care plan.
  • Assists the team in engaging the patient in holistic services and follow-up appointments.
  • Orders supplies and maintains supply inventory.
  • Cleans and stocks exam rooms; sterilizes equipment and instruments.
  • Performs routine testing of equipment and compliance duties.
  • Provides information and education to patients and their families on chronic disease management, health and wellness, and preventative measures to ensure adequate awareness of needs and recommends appropriate community resources as needed.
  • Follows clinic workflows and suggests opportunities for improved patient care.
  • Performs other duties as assigned.

Minimum Qualifications:

  • Successful completion of Associate Degree RN (ADN) or LPN required from an accredited program.
  • Licensed in the state of Nebraska as an RN or LPN required.
  • Minimum two (2) years of successful work experience as an RN or LPN in a clinic required; mental health clinic experience preferred.
  • Current BLS certificate required.
  • Demonstrated organizational skills, dependability in attendance and timeliness, and ability to be flexible and adaptable to changing work conditions.
  • Strong written and verbal communication skills.
  • Demonstrated ability to work as part of a team and consistently present a welcoming, professional, and customer-oriented environment.
