Overview
BAYADA Home Health Care is seeking a Home Health Physical Therapist for a position in Hatboro, Pennsylvania. The office serves adult and geriatric clients on a per-visit basis throughout Bucks County and Northeast Philadelphia. Weekend schedule options:
Friday - Sunday or Saturday - Monday. Home health experience required.
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements (mobility/range of joints, transfer status, stability, ambulation patterns and appearance, muscle strength and endurance, balance testing, safety assessments).
- Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continuously assess and revise the PT care plan; participate with nursing in the multidisciplinary care plan as appropriate.
- Educate and instruct clients, family members, or other representatives in rehabilitative care to promote health, safety, and independent living.
- Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
- Current PA Physical Therapist - PT license.
- Minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
