Social Worker
We’re seeking a compassionate and motivated Social Worker to join our dedicated team. In this role, you’ll play a key part in supporting the emotional and psychosocial well-being of our residents, working closely with families, interdisciplinary team members, and community partners.
Social Worker Responsibilities:- Assist residents and families in adjusting to short-term rehab or long-term care placement
- Conduct psychosocial assessments and care planning
- Coordinate discharge planning and referrals
- Advocate for residents’ rights and help resolve concerns
- Participate in care plan meetings and interdisciplinary team discussions
- Maintain accurate documentation in compliance with state and federal regulations
- Bachelor’s or Master’s degree in Social Work (BSW or MSW)
- Prior experience in long-term care or healthcare setting preferred
- Strong interpersonal, organizational, and problem-solving skills
- Knowledge of federal and state regulations related to nursing facilities
- Empathetic, professional, and resident-centered approach
