Job Overview – Program Specialist (Temporary)
Compensation: $35/hour |

Schedule:
Monday to Friday, 7:30 AM – 4:00 PM
TAG Med Staffing is hiring a Program Specialist (Temporary) in Montgomery County, PA for our client, supporting care coordination, program oversight, and individualized services for individuals with intellectual disabilities and autism. This contract role will lead interdisciplinary teams, coordinate individualized service plans, and help ensure services promote community inclusion, independence, and person-centered care. The ideal candidate will have experience supporting individuals with intellectual and developmental disabilities, strong organizational skills, and a collaborative approach to service coordination.
Responsibilitiesas the Program Specialist (Temporary)
- Program Coordination: Lead interdisciplinary teams to develop, implement, and monitor individualized service plans that support person-centered goals and community inclusion.
- Care Management: Coordinate services with individuals, families, providers, and community resources to ensure appropriate support and continuity of care.
- Documentation & Compliance: Maintain accurate service documentation, monitor program compliance, and ensure individualized plans are current and implemented appropriately.
- Consumer Advocacy: Promote self-advocacy, independence, and quality of life while ensuring services align with individual needs and preferences.
- Interdisciplinary
Collaboration:
Partner with internal teams, service providers, and external stakeholders to resolve issues, communicate updates, and support positive outcomes. - Quality Improvement: Monitor service delivery, identify opportunities for improvement, and assist with corrective action plans and program initiatives as needed.
- Education: Bachelor’s degree in Human Services, Social Work, Psychology, Education, or a related field is preferred. Equivalent combinations of education and relevant experience will be considered.
- Experience: Experience in case management, program coordination, social services, human services, behavioral health, disability services, or a related field is required.
- Industry Knowledge: Knowledge of person-centered services, care coordination, community-based programs, or social services is preferred.
- Technical
Skills:
Proficiency with Microsoft Office Suite and experience with electronic documentation, case management, or related software is preferred. - Skills & Attributes: Strong communication, organizational, problem-solving, and interpersonal skills with the ability to manage multiple priorities and collaborate effectively across multidisciplinary teams are required.
