• Conduct and participate in quarterly quality improvement meetings, review data, identify trends, and oversee implementation of improvement plans.
• Collaborate with the Branch Director to implement quality improvement activities and corrective action plans.
• Oversee response to external and internal surveys and audits on quality of care and clinical KPIs.
• Complete documentation and chart reviews to identify variances and areas for improved performance.
• Hold case conferences to ensure oversight of care and coordination of services.
• Facilitate development of performance improvement action plans, communicate job expectations, plan, monitor, and appraise results, coach, counsel, discipline employees, and enforce systems, policies, procedures, and productivity standards.
• Ensure staff are evaluated fairly and timely in accordance with company performance evaluation procedures.

• Bachelor’s Degree preferred in Nursing from an accredited school of nursing.
• Licensed Registered Nurse (RN) in good standing and currently licensed by the State.
• Less than two years of clinical experience, and at least one year in a related management/supervisory role in home health care.
• Current CPR certification.
• Must meet all agency requirements for pre‑employment as required by the company and/or State regulations.
• Ability to use company documentation systems.
• Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company.
• Ability to work independently as well as part of a team.
• Capable of working responsibly with highly confidential information.
