Licensed Practical Nurse (LPN) to support clients’ health, safety, and recovery in a behavioral health and substance use treatment setting. Provide compassionate and clinical care, medication support, monitor health, document and coordinate care, and collaborate with clinical and program leadership for safe, effective, compassionate care.
Responsibilities- Provide direct nursing care to clients under the supervision and direction of clinical leadership.
- Maintain patient confidentiality in accordance with Phoenix House policies and procedures.
- Oversee unlicensed assistive personnel (e.g., CNAs, Medical Assistants, Medical Technicians).
- Perform routine nursing procedures such as EKGs, wound care, and specimen collection; administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal) in accordance with NYS scope of practice and facility policies.
- Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership.
- Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership.
- Engage in health education and reinforce treatment plans, medication adherence, and self‑care strategies.
- Other duties as assigned.
- High School Diploma or GED.
- Graduate of an accredited nursing program.
- Current NYS Licensed Practical Nurse.
- First Aid Training.
- CPR Training.
- Narcan Training.
- Infection Control Certification.
- Child Abuse Certification.
- At least three years of related experience.
- Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and/or drugs.

This job operates in an indoor clinical environment. The role routinely encounters patients who may have contagious illnesses.
Physical Demands- Regularly required to stand, walk, use hands to type, handle or feel objects, tools, or controls; reach with hands and arms, and talk or hear.
