Job Definition

The Registered Nurse will work with the CN Guidance's Mobile Residential Support Team and all Community Residence Programs to deliver services to individuals across Nassau and Suffolk County living in all levels of housing who are diagnosed with mental illness and require additional supports in order to live successfully in the community. The Registered Nurse provides direct patient care and implements treatment plans under the direction of the Nurse Manager and medical staff, following established clinical guidelines, policies, and procedures.

Educational

Requirements

Must be currently licensed as a Registered Nurse by the New York State Department of Education and possess either an Associate's Degree (if awarded prior to December 18, 2017), Bachelor's Degree and/or Master's Degree in Nursing. Bachelor's or Master's Degree preferred.

Experience Required

One (1) year of community mental health experience and/or at least two (2) years of mental health experience specifically working in a mental health setting.

Duties and Responsibilities
  • Responsible for the coordination of client services to include scheduling of outside clinic appointments, lab appointments, obtaining random urine analysis (UA), and intramuscular (IM) and subcutaneous injection administration, when needed.
  • Conduct medication reconciliation, when appropriate, or during transitions of care.
  • Conduct vitals and health assessments.
  • Ensure the completion of all medical assessments for each client, including coordinating additional services with the psychiatrist or medical provider.
  • Monitor the pharmacological effects of prescribed psychotropic and medical medications and notify the psychiatrist or medical provider of their reported effectiveness.
  • Work in conjunction with the medical provider, psychiatrist and nurse practitioner, responsible for coordinating patient education efforts to include the benefits, risks and possible side effects of prescribed medication. This education effort is not limited to home visits but may also include the coordination of various groups to include medication, wellness, etc.
  • Monitor the client's medication regimen to ensure completeness and accuracy in the provision of care and documentation in the agency electronic medical record.
  • Assist the medical provider, psychiatrist and/or nurse practitioner by scheduling clients for visits based upon client needs, post‑hospitalization and/or incarceration. The RN will also advise the medical provider, psychiatrist or nurse practitioner of all medication renewal needs.
  • Be available (along with other members of the Mobile Residential Support Team and Crisis Retreat) on an as needed basis twenty‑four (24) hours a day, seven (7) days a week, including responsibility for periodic on‑site coverage.
  • Enter the documentation for each client visit in the electronic health record as instructed by the program supervisory/management team within twenty‑four (24) to seventy‑two (72) hours after the client visit.
  • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision‑making and to keep clients informed of progress.
  • Complete QI training during onboarding and participate in QI refresher training as needed.
  • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
  • Adhere to all safety protocols and procedures to ensure a safe working environment for all employees.
  • Maintain health and safety awareness regarding relevant protocols.
  • Other duties as assigned.

Full‑time counselor will receive a $750 bonus after 60 days and an additional $1,000 bonus after 8 months.
