Sunrise Senior Living - - - Responsibilities:
Deliver care and services for designated guests/residents according to Individualized Service Plans (ISP).;
Perform individualized personal care and activities of daily living (ADL); documents ADL in flow sheets.;
Partner with licensed nurse to plan, develop, organize, provide and execute individualized restorative therapy programs, activities and standards for their designated guests/residents.;
Obtain, document and report the guest/residents vital signs, urine output and meal intake per individualized care plan to licensed nurse as requested.;
Maintain Restorative Records for each guests/residents.
