Haverhill, MA (Day Habilitation Program)

Schedule:

Monday – Friday 8:00am – 4:00pm

Compensation: $55,000 – $57,000 per year; 40‑hour, full‑time, non‑exempt. Eligible for benefits from day of hire.

Job Responsibilities
  • Hire, supervise, evaluate, and train Case Managers and Developmental Specialists.
  • Create and implement curriculums and activities that support the interests and needs of individuals served, their ISP goals, and comply with State, Federal, and agency regulations.
  • Coordinate monthly Interdisciplinary Team Meetings to ensure clinical (OT, PT, SLP), medical and behavioral support services are provided to coincide with identified needs, including training and proper sign‑offs.
  • Ensure timely submission of program data for individuals receiving services.
  • Support the development, implementation, and ongoing review of Day Habilitation Service Plans (DHSPs), Individual Support Plans (ISPs), and Behavior Support Guidelines and ensure all goals and objectives are clinically appropriate, person‑centered, and aligned with assessments.
  • Monitor and reinforce consistent implementation of approved strategies and interventions to promote positive outcomes, regulatory compliance, and continuity of care.
  • Coordinate referral and intake processes, including tours, documentation, funding communication, and finance notifications.
  • Maintain knowledge of and ensure compliance with CARF, OQE, DDS, MH, ABI, and agency policies.
  • Partner with the Director and Community Engagement Coordinator to ensure services are member‑driven and aligned with Positive Behavior Supports.
  • Assist with development and monitoring of the annual department budget.
  • Oversee facility safety, cleanliness, inventory, and serve as liaison with families/guardians to promote community involvement.
  • Participate in inspections, audits, quality reviews, newsletters, and complete all required trainings and certifications.
Qualifications
  • Bachelor's degree in Human Services or related field with at least 3 years of experience within /DD, or High School Diploma/GED with at least 5 years of experience within /DD.
  • Bilingual proficiency in English and Spanish preferred.
  • Minimum of 2 years of supervisory experience.
  • Minimum of 2 years of experience implementing, developing, and monitoring Individual Service Plans or similar plans.
  • Familiarity with the Positive Behavior Supports model by DDS a plus.
  • Experience working with Electronic Health Records (EHR) platforms such as eHana.
  • Strong communication, organizational, and interpersonal skills with ability to manage multiple priorities in a fast‑paced environment.
  • Proficient with Microsoft Office and general office technology; able to handle confidential information with professionalism and sound judgment.
  • Valid Driver’s license with at least 1 year’s driving experience required.
  • Regular and predictable attendance.
