LVN​/LPN – LTC​/SNF

Job Summary:

Provides skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensures resident wellbeing through medication administration, treatments, and assessments, adhering to facility policies and state regulations.

Responsibilities:

• Monitor and report residents' symptoms and condition changes.
• Administer medications and treatments as prescribed by physicians.
• Maintain accurate documentation of resident care, medication, and progress.
• Assist with physician orders, resident assessments, MDS, and care plans.
• Supervise CNA staff, coordinate assignments, and perform direct resident care as needed.
• Call physicians, schedule tests, labs, and coordinate resident admissions and discharges.

Qualifications:

• Current licensure as an LVN or LPN in Massachusetts.
• Graduate of an approved nursing program.
• Ability to recognize clinical changes and respond accordingly.
• Proficient in reading, writing, and communicating effectively in English.
• Current CPR/AED certification.
• Ability to lift up to fifty pounds and stand for extended periods.
