LVN​/LPN – LTC​/SNF

Job Summary:

Provides nursing care to residents in a long-term care or skilled nursing facility setting, ensuring adherence to healthcare standards and protocols. Supports the interdisciplinary team in delivering comprehensive resident-centered care.

Responsibilities:

• Observe and document residents' symptoms and overall health status.
• Administer medications and treatments accurately as prescribed by physicians.
• Transcribe and communicate physician orders effectively.
• Assist with resident admissions, discharges, and ongoing assessments.
• Supervise and coordinate CNA activities and care assignments.
• Maintain detailed records of resident care, medications, and progress.

Qualifications:

• Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in Massachusetts.
• Graduate of an accredited nursing school.
• Valid CPR/AED certification.
• Ability to recognize changes in residents' conditions and respond appropriately.
• Excellent communication skills in English, both written and verbal.
