New England Center and Home for Veterans seeks a SSVF Stabilization Case Manager to support Merrimack Valley veterans facing homelessness. You will conduct outreach, intake and assessment, and coordinate housing plans with landlords and service partners.
You’ll connect Veterans to benefits, rapid housing, and stabilization services, maintain documentation, attend team meetings, and travel within the Merrimack Valley as needed.
