Waystone is currently looking for a dedicated Case Manager based out of our Day Habilitation Program in Haverhill. As a Case Manager, you will oversee and coordinate all support services for individuals served by Waystone. This includes developing service plans, implementing goals, conducting assessments, maintaining accurate records, ensuring documentation compliance, tracking attendance, and completing all required submissions through Electronic Health Records and related systems.

The Case Manager also trains and guides staff to ensure support plans are carried out effectively and that high‑quality services are consistently delivered.

Location: Haverhill, MA (Day Habilitation Program)

Schedule: Monday – Friday 8:00am – 4:00pm. This position is benefit eligible as of date of hire.

Compensation: This is a 40 hour, non-exempt, full-time position. The hourly rate is $23.69. You will be a union member to SEIU Local 509.

Case Management
  • Coordinate services aligned with DHSP/ISP and Behavior Support Plans to promote choice, inclusion, dignity, independence, and skill development.
  • Document meetings for case conferences, support plan reviews, and trainings.
  • Identify concerns proactively and collaborate with Supervisor/Director for person‑centered solutions.
  • Provide IDT updates with data‑informed feedback on medical status, goal progress, and safety concerns.
  • Implement plans including crisis plans, behavior treatment plans, and medical protocols.
Service Delivery
  • Provide personal care aligned with medical, physical, and psychological needs.
  • Understand individual profiles to support positive learning environments.
  • Communicate issues affecting health, safety, or well‑being.
  • Collaborate with teams including staff, families, departments, and funding sources.
  • Develop curriculum aligned with day habilitation need areas.
  • Advocate for community engagement in recreational, vocational, and therapeutic activities.
  • Incorporate feedback to support decision‑making and skill development.
Compliance
  • Maintain documentation including attendance, incidents, accidents, restraints, case notes, and health records.
  • Support DHSP/ISP goals and ensure data integrity across electronic systems.
  • Assist with audits including binder audits and training compliance.
Other Responsibilities
  • Promote teamwork and provide person‑specific training/support.
  • Respect rights in all interactions.
  • Maintain confidentiality for all individual information.
  • Complete mandatory training including CPR, First Aid, and Waystone‑required courses.
  • Report injuries immediately and complete follow‑up documentation; ensure Mass Health compliance.
  • Participate in QA inspections, audits, and reviews.
  • Contribute to newsletter with team collaboration.
  • Provide transportation as requested.
  • Maintain safe environment by reporting issues promptly.
Qualifications
  • Bachelor’s degree in Human Services or related field with at least three (3) years of experience within the field of /DD or High School Diploma or GED with at least five (5) years of experience within the field of /DD.
  • Bilingual proficiency in English and Spanish preferred.
  • Minimum of two (2) years with case management, case reviews, development and implementation of goals and group activities, preferably in a related setting.
  • Experience working with adults with developmental disabilities for a minimum of 1 year required.
  • Familiarity with the Positive Behavior Supports model by DDS is a plus.
  • Strong communication, organizational, and interpersonal skills with the ability to manage multiple priorities in a fast‑paced environment.
  • Proficient with Microsoft Office and general office technology; able to handle confidential information with professionalism and sound judgment.
  • Valid Driver’s license with a minimum of 1 year’s driving experience required.
  • Regular and predictable attendance.
