The Emerald Peek Rehabilitation and Nursing Center seeks a compassionate Licensed Practical Nurse to deliver resident-centered care in our skilled nursing and short-term rehabilitation facility. The LPN provides hands-on clinical care, including medication administration, wound care, vital sign monitoring, and accurate documentation, while supporting recovery, long-term care, and memory care residents. Working in an interdisciplinary, collaborative environment, the LPN contributes to care planning, educates residents and families, and upholds infection control and safety standards.

Responsibilities
- Provide direct nursing care to residents in rehab, long-term care, and memory care units under RN and physician direction
- Administer medications and treatments, including wound care and monitoring IVs per scope of practice
- Monitor, document, and report changes in resident condition using EHR systems
- Participate in care planning with the interdisciplinary team and support discharge planning and recovery goals
- Educate residents and families on care plans, medications, and wellness strategies
- Support daily living activities with a focus on dignity, safety, and resident-centered care
- Follow infection control, safety, and regulatory standards in all clinical activities
- Precept, support, and collaborate with CNAs and other team members to ensure quality care
