Works Under The Supervision Of The Nurse Manager/Clinical Coordinator.

Responsibilities
  • Assess, plan, implement, evaluate, and supervise individual patient care on a nursing unit/department according to unit policies and procedures.
  • Provide supervision of non-RN personnel on the assigned unit.
  • When assuming the charge role, delegate and supervise unit activities.
  • May be required to work on other nursing units according to staff distribution and patient need.
  • Perform other duties as assigned or requested to maintain a high level of service.
  • Assess the patient’s physical and psychosocial condition on an ongoing basis throughout the continuum of care.
  • Develop, communicate, and document a plan of care reflecting individualized patient problems and interventions to achieve desired outcomes, involving the patient, family, and healthcare team.
  • Implement and coordinate the plan of care in collaboration with the healthcare team.
  • Evaluate the effectiveness of the plan of care, the patient’s response to care, and readiness for discharge.
  • Revise the plan of care as needed and make referrals where indicated.
  • Plan for education after assessing the individual learning needs and readiness of the patient and family.
  • Provide educational opportunities and document outcomes.
  • Act as a preceptor for new staff.
  • Act as a charge nurse.
  • Meet performance expectations for customer service, teamwork, resource utilization, and staff and self-development as outlined in performance reviews.
  • Perform other duties as assigned or directed to ensure smooth operation of the department/unit.
  • Demonstrate competency in caring for neuro patients, including assessment, airway management, care of a trached patient, assisting physicians during special neuro procedures, teaching, pharyngeal and tracheostomy suctioning, use of equipment and care of patients in a halo brace, and nursing responsibilities for ventilator patients.
Qualifications
  • Graduate of an accredited school of nursing; BSN preferred.
  • Proficient in use of computer software, including Electronic Patient Records.
  • Ability to use the internet for literature searches regarding patient care.
  • At least 1 year of acute nursing experience.
  • Valid State of Connecticut Registered Nurse license.
  • BLS required.
  • Critical Care or Emergency experience preferred.
