Description

The Speech Language Pathologist (SLP) will carry out an individualized program of Speech Language Pathology including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The SLP will also maintain a positive working relationship with other health professionals and ancillary staff. This position requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential.

The individual shall perform all duties in a manner that promotes team concept and is reflective of Kahuku Medical Center's mission.

Supervisory Responsibilities
  • Potential supervision of support staff including aides and/or assistants
  • Case management of patient caseload
Duties and Responsibilities
  • Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients' functional needs and adjusts care plans to ensure optimal patient outcomes.
  • Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
  • Conducts therapy practice within defined standards of care.
  • Directs the assistance of support staff to achieve patients' functional goals.
  • Communicates effectively via written, verbal, and digital communication with other professionals and support staff to achieve positive patient outcome.
  • Practices hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.
  • Efficiently documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures. Documentation is to be completed in the Hospital's electronic medical record system.
  • Documentation shall be completed within the established parameters of the department.
  • Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.
  • Provides direction and leadership to staff and acts as a resource to promote Speech Language Pathology in the community.
  • Punctuality and professionalism are key attributes to the position. The expectations is to arrive for shift on time and ready to receive the first patient on time. The expectation is also to make all preparations to leave on time at the end of the day. Overtime will not be granted unless previously approved.
  • Strict adherence to all department and Hospital policies and procedures.
  • Perform other duties as assigned.
Required Knowledge/Skills/Abilities
  • Knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services.
  • Ability to remain calm in emergency or crisis situations.
  • Flexibility is a key aspect of the position. The therapist must be willing to be flexible during patient care and willing to work with the hospital team. The therapist may be required to flex between hospital and outpatient care.
  • Documentation
    • Efficient and effective documentation is a requirement of the position. A therapist shall complete documentation during the therapy day. The quality of documentation shall also reflect an appropriate subjective report, education provided, assessment of the treatment, plan for the next session, and appropriate charge delineation.
    • It is expected that documentation of patient appointment, daily notes, and evaluation be initiated point of contact.
    • Evaluation and daily notes shall be completed on the same day of service.
    • All notes will be completed in the hospital's EMR.
