Pay Range

Starting at $24.15/hour

Help patients overcome barriers to care, connect to community resources, and reach their health goals—all while making a real impact across Hawaiʻi Island.

You’ll work alongside a supportive interdisciplinary team in a value‑based care model designed to improve health outcomes, strengthen patient engagement, and reduce avoidable ER visits and hospitalizations. Translation? Your work truly matters.

What We’re Looking For
  • Required: High School Diploma or GED
  • At least 2 years of experience in health or social services
  • Strong knowledge of community resources
  • Valid Hawaiʻi driver’s license, insurance, and reliable transportation
  • Preferred: Bachelor’s degree in a human services‑related field
