Occupational Therapist Position

Seeking a licensed Occupational Therapist to provide comprehensive therapy services in a skilled nursing facility setting. The role involves evaluating patients, developing treatment plans, and implementing therapies aimed at rehabilitation and habilitation.

Responsibilities:

• Conduct diagnostic assessments to determine patients’ functional abilities and treatment needs.
• Develop and implement individualized occupation-based treatment plans.
• Utilize EMR systems to document patient progress and treatment outcomes.
• Collaborate with multidisciplinary teams to coordinate patient care.
• Offer therapy services during scheduled shifts, including weekends and holidays as needed.
• Maintain current licensure and certifications to comply with state regulations.

Qualifications:

• Hold an active Hawaii Occupational Therapy License before start date.
• Minimum of two years’ experience working with adults in acute, SNF, rehab hospital, or outpatient settings.
• Demonstrate proficiency with EMR software, preferably Meditech.
• Possess current BLS certification.
• Strong clinical assessment, treatment planning, and implementation skills.
• Ability to work in a fast-paced environment with flexible scheduling.
