The Community Health Worker (CHW) will assist LCHC Providers and clinical staff in providing quality health care. The CHW will be responsible for helping patients and their families to navigate and access community services, other resources, and implement healthy behaviors. The CHW supports providers through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain, and improve the health of patients and their family.

Provide social support and informal counseling, advocate for individuals and community health needs. The CHW will also provide health screenings and home visits.

Responsibilities
  • Support patients across medical, dental, vision, behavioral health, and specialty services
  • Build trusting relationships with patients and families
  • Conduct intake interviews and prepare patients for provider visits
  • Provide follow-up via phone, home visits, and community outreach
  • Assist with applications, registration, and access to services
  • Coordinate care plans, referrals, and appointments
  • Collaborate with providers and care teams to ensure continuity of care
  • Advocate for patients and connect them with community resources
  • Perform basic clinical tasks (e.g., vital signs, glucose checks)
  • Document patient interactions accurately in the EHR within 24 hours
  • Support patient transportation needs as appropriate
  • Follow all policies, procedures, and safety protocols
  • Assist with health education and interdepartmental initiatives
  • Perform other duties as assigned
Qualifications
  • High school diploma or equivalent; bachelor’s degree in a related field is preferred.
  • Proven experience in community outreach, public health, or related fields.
  • Strong communication and interpersonal skills.
  • Knowledge of local healthcare resources and social service programs.
  • Ability to work effectively with individuals from diverse backgrounds.
  • Compassionate, empathetic, and culturally sensitive approach to healthcare advocacy.
Additional Requirements
  • Certification as a Community Health Worker is a plus.
  • Bilingual proficiency in languages commonly spoken in the community is advantageous.
