Job Summary:

Provide comprehensive physical therapy evaluations and treatments across inpatient, outpatient, ICU, and SNF settings. Deliver quality care with a focus on patient mobility, discharge planning, and individualized therapy plans in a rural healthcare environment.

Responsibilities:

• Assess patients’ physical conditions to develop and implement personalized treatment plans.
• Perform therapeutic interventions in inpatient, outpatient, ICU, and SNF settings.
• Manage mobility progression from acute to SNF care, ensuring patient safety and functional improvement.
• Collaborate with healthcare teams to coordinate discharge planning and patient care.
• Maintain accurate documentation of evaluations, treatments, and progress notes.
• Adhere to all safety protocols and adhere to professional standards of practice.

Qualifications:

• Hold an active Hawaii PT license or be able to obtain it promptly.
• Demonstrate proficiency in inpatient, outpatient, ICU, and SNF therapy techniques.
• Possess strong assessment and treatment planning skills.
• Have excellent communication and collaboration abilities with multidisciplinary teams.
• Exhibit independence and thoroughness in patient care, especially in rural settings.
• Previous experience in acute care, ICU, and outpatient therapy is preferred.
