Physical Therapist (PT) – Home Health | Waianae, HI | Part-Time & PRN | $65–$85 Per Visit

Stern at Home Therapy is hiring Part-Time and PRN Physical Therapists in Waianae, HI to provide one-on-one care to patients in their homes.

This is a great opportunity for a PT who wants to build a consistent local caseload or add flexible visits around an existing schedule without the double-booking, productivity quotas, or clinic politics common in traditional outpatient settings.

As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population in their homes throughout the Waianae area
. You’ll have the clinical autonomy to develop individualized plans of care while our administrative team supports you with scheduling, referrals, authorizations, and patient coordination.

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one physical therapy treatments in patients' homes.
• Help patients improve strength, balance, mobility, endurance, and functional independence.
• Provide gait training, transfer training, fall-prevention strategies, and home exercise programs.
• Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
• Complete accurate and timely clinical documentation.
• Communicate with physicians, caregivers, families, and members of the interdisciplinary care team.
• Manage your caseload around your availability.

• $65 per follow-up visit
• $75 per 4-unit evaluation

• $72 per follow-up visit
• $85 per evaluation

• $72 per follow-up visit
• $85 per evaluation

Part-time and PRN compensation is paid at a fixed per-visit rate and does not use a tiered volume model.

Eligible Part-Time W-2 employees may receive:

• Active Physical Therapist license in Hawaii required.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation required.
• Strong clinical, communication, and documentation skills.
• Ability to work independently in a home-based setting.
• Home health or geriatric experience is preferred but not required.
