Job Description

Job Summary:

Provide direct patient care in a long-term care or skilled nursing facility setting, supporting nursing staff with a variety of patient assistance tasks. Ensure the safety and comfort of patients while adhering to established care protocols.

Responsibilities:

• Assist patients with activities of daily living, including bathing, dressing, and mobility.
• Monitor and report patient condition changes to nursing staff.
• Maintain a clean and safe environment in accordance with facility policies.
• Support medication reminders and assist with basic treatments as instructed.
• Document patient care activities accurately and promptly.
• Work flexible shifts, including nights, weekends, and holidays, as needed.

Qualifications:

• Valid CNA certification required.
• Proven experience in a long-term care, SNF, or home care environment.
• Ability to work any shift and adapt to rotating schedules.
• Strong communication and interpersonal skills.
• Ability to lift, move, and assist patients safely.
• Demonstrate compassion and professionalism in patient care.
