Licensed Practical Nurse (LPN) Long-Term Care Position Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) for a contract assignment in a Long-Term Care (LTC) setting. The ideal candidate will have experience providing direct patient care, administering medications, and supporting residents with a variety of healthcare needs while maintaining a high standard of quality and compassion.
Schedule- Varied 8-hour shifts
- Days: 7:30 AM 3:30 PM
- Evenings: 3:00 PM 11:30 PM
- Nights: 11:00 PM 7:30 AM
- Weekend and holiday coverage required
- Overtime rotation may be required
- No guarantee of hours or shift preference
- Administer medications and treatments as prescribed
- Perform resident assessments and monitor changes in condition
- Provide skin and wound care treatments
- Assist residents with activities of daily living (ADLs)
- Administer nebulizer treatments
- Manage and monitor wound vacuum (Wound Vac) therapy
- Provide CPAP care and monitoring
- Perform gastrostomy tube (G-tube) care and feeding support
- Accurately document patient care and treatment plans
- Collaborate with physicians, registered nurses, and interdisciplinary care teams
- Ensure compliance with facility policies, safety standards, and infection control practices
- Active Hawaii LPN License required by assignment start date
- Minimum of 1 year of nursing experience preferred
- Long-Term Care experience preferred
- Current BLS/BCLS certification required
- Medication administration proficiency
- Resident assessment and monitoring
- Wound care and skin treatment experience
- Strong communication and interpersonal skills
- Ability to work independently and as part of a healthcare team
- Compassionate, patient-centered approach to care
- Strong organizational and documentation skills
- Experience with EPIC electronic medical records (EMR) preferred
- Proof of current influenza vaccination or an approved medical/religious exemption is required prior to the start of the assignment.
- Candidates without a Hawaii nursing license at the time of application must obtain licensure prior to the assignment start date.
- Candidates must be available to complete pre-employment health clearance requirements prior to the start of the assignment.
- Flexibility to work varying shifts, weekends, holidays, and overtime as needed is required.
