LVN ​/ LPN – LTC​/SNF

Call for Salary Rate -

Job Summary:

Provide nursing care to patients in a long-term care or skilled nursing facility setting. Ensure patient comfort, safety, and adherence to care plans under the supervision of registered nurses and physicians.

Responsibilities:

• Administer medications and treatments as prescribed by healthcare providers.
• Perform skin assessments and deliver skin care treatments.
• Assist patients with Activities of Daily Living (ADLs), including bathing, grooming, and mobility.
• Conduct nebulized treatments and manage wound vac care.
• Provide CPAP and gastrostomy tube care, ensuring proper device functionality.
• Document patient observations and care provided accurately and timely.

Qualifications:

• Valid Hawaii Nursing License required before start date.
• Proven experience with medication administration and patient assessments.
• Ability to perform wound care, skin treatments, and use respiratory equipment.
• Commitment to patient safety and quality of care.
• Flexible schedule, including availability for varied shifts, weekends, and holidays.
• Ability to handle OT rotations and adapt to changing shift requirements.
