CNA – LTC​/SNF

Job Summary:

Provides direct patient care in long-term care and skilled nursing facilities, assisting with daily living activities and clinical tasks to support patient well-being. Flexible to work various shifts and care units, including home care as needed.

Responsibilities:

• Assist patients with personal hygiene, mobility, and daily activities.
• Monitor and report patient conditions to nursing staff.
• Perform basic clinical tasks such as vital signs and assisted procedures.
• Support nursing staff in delivering comprehensive patient care.
• Adapt to different care environments, including SNF, ICF, and home settings.
• Follow established procedures to ensure patient safety and comfort.

Qualifications:

• Valid CNA certification required.
• Proven experience in long-term care or skilled nursing facilities preferred.
• Ability to work flexible shifts, including nights and weekends.
• Strong communication and patient care skills.
• Ability to perform clinical tasks accurately and efficiently.
• Demonstrates professionalism and compassionate patient interaction.
