In-Home Nurse Practitioner; Part Time - Hays, KS

Signify Health, part of CVS Health®, is seeking a Part‑time Nurse Practitioner to provide In‑Home Health Evaluations.

In this role, you will conduct personalized in‑home visits, perform comprehensive physical evaluations, review medication and medical histories, and place diagnostic tests as ordered by the member’s health plan. Visits last approximately 45 minutes and may require travel within licensed states. You will use an iPad and adhere to an intuitive clinical workflow. You are not required to prescribe medication or order laboratory tests.

• Build and maintain trusted relationships with patients in their homes.
• Deliver comprehensive health evaluations and connect patients to appropriate care.
• Travel locally and within licensed states as needed and conduct virtual visits when necessary.
• Provide Diagnostic and Preventive Services, including spirometry and diabetic eye exams.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
