Nursing Assistant, Certified, Long Term Care; LTC

Overview

The Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long‑term care, providing resident‑centered nursing care and daily living assistance under the supervision of a registered nurse (RN).

• Provides basic health care needs including bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and offering psychosocial support to assigned residents.
• Assists residents with transferring, repositioning, and walking using correct transfer techniques and equipment, and provides range of motion and passive exercises.
• Prepares food, assists residents with meals and snacks, and provides water and supplements as appropriate, fostering a positive dining experience.
• Supports providers and nursing staff with procedures, when needed.
• Documents resident interactions and outcomes related to care, behavior, activity, and diet.
• Communicates changes in resident condition and care concerns/responses to the charge nurse.
• Collects non‑invasive body fluid specimens and gathers vital signs.
• Performs housekeeping and laundry services to help residents maintain clean living areas.
• Assists with wellness and recreational programs, encouraging resident participation.
• May participate in departmental or organization committees and task forces as required.
• Adheres to infection prevention and control policies and uses personal protective equipment (PPE) appropriately.

High‑school graduate or GED preferred (not required). Minimum age 16. Prior clinical or nursing assistant experience in long‑term care preferred.

Required:

Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state‑certified CNA within four months of starting the position, either through enrollment in a training program or in the process of obtaining certification via endorsement. The facility provides CNA training and certification.

Must satisfactorily complete yearly department‑specific competency testing and maintain a record of continuing education credits annually. A minimum of 12 hours of continuing education is required each year. Maintains all department‑specific required certifications, and when applicable, is certified and registered with the Unlicensed Assistive Person (UAP) Registry.

Depending on facility responsibilities, this role may classify as a Sanford Category II Driver, requiring a motor vehicle report and proof of a valid driver’s license, and medical qualification for essential driving functions per Sanford Driving Policy.
