Job TitleFull-time position - Van Driver and CNA

- Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
- Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
- Observe the following guidelines at each visit:
- Sign in resident upon entrance.
- Present resident’s documents to office personnel.
- Before leaving doctor’s office, please observe the following:
- Obtain resident’s documents.
- Obtain any new prescriptions.
- Check to make sure doctor has written on the doctor’s order form.
- If follow-up appointment necessary, obtain appointment cards.
- After returning to facility, report to RCM and be prepared to answer questions concerning visit.
- Performs other duties as required and assigned.
- Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
- Possess a valid state driver’s license.
- Qualified as a Certified Nurse Assistant (C.N.A.).
- Clean driving record.
- Clean criminal record.
- Any other state required certifications.
