• Assess patients' functional performance and rehabilitation needs with thoughtful evaluations
• Develop personalized treatment plans that prioritize patient goals and independence
• Implement therapeutic activities and adaptive strategies to maximize daily living skills
• Monitor and document patient progress following state and federal guidelines
• Educate patients and families on therapy goals, home programs, and adaptive equipment
• Collaborate closely with nurses, PTs, physicians, speech therapists, and the interdisciplinary team to coordinate care

• Active and current Occupational Therapist (OT) license in the state of practice (required)
• Graduate of an accredited Occupational Therapy program (OTD or equivalent)
• Previous experience in a Skilled Nursing Facility (SNF) is required
• Strong clinical knowledge with a patient-centered approach
• Excellent communication, organization, and teamwork skills
