Are you an Occupational Therapist passionate about helping residents regain their independence and quality of life? Hayward Gardens Post Acute is looking for a dedicated Full‑Time OT to join our elite in‑house therapy team. In this role, you will play a critical part in our residents' recovery, focusing on functional daily living skills and personalized therapeutic interventions.

Your Impact & Responsibilities:
  • Functional Assessment: Conduct comprehensive evaluations of residents' activities of daily living (ADLs), cognitive function, and physical capabilities.
  • Goal‑Oriented Planning: Create individualized treatment plans focused on restoring independence in self‑care, mobility, and community reintegration.
  • Therapeutic Intervention: Provide hands‑on therapy, including upper extremity strengthening, fine motor coordination, and adaptive equipment training.
  • Home Safety & Discharge Planning: Assess resident needs for a safe transition home, including recommending home modifications and providing family education.
  • Interdisciplinary Care: Collaborate closely with the nursing and dietary teams to ensure a holistic approach to resident wellness and safety.
  • Clinical Documentation: Maintain accurate and timely electronic health records in compliance with state and federal regulations.
Qualifications:
  • License: Must possess a current, valid California Occupational Therapist (OT) license.
  • Experience: Previous experience in a skilled nursing, post‑acute, or rehabilitation setting is preferred, but we welcome passionate clinicians of all experience levels.
