Job SummaryWe are seeking a dedicated Occupational Therapist (OT) to provide rehabilitation services in a Skilled Nursing Facility (SNF) setting. The Occupational Therapist will work with patients recovering from illness, injury, or surgery, focusing on improving functional independence and quality of life through therapeutic interventions and individualized treatment plans.
Key Responsibilities- Evaluate patients’ physical, cognitive, and functional abilities to develop individualized treatment plans.
- Provide therapeutic interventions aimed at improving daily living skills, mobility, strength, coordination, and independence.
- Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals to coordinate patient care.
- Educate patients and caregivers on adaptive techniques, assistive devices, and home exercise programs.
- Monitor patient progress and modify treatment plans based on clinical outcomes.
- Maintain accurate and timely patient documentation in accordance with facility policies and regulatory requirements.
- Participate in interdisciplinary care planning and discharge planning as appropriate.
- Ensure compliance with professional standards, safety protocols, and rehabilitation best practices.
- Active Occupational Therapist license in the state of practice.
- Minimum of one year of occupational therapy experience, preferably in a skilled nursing or rehabilitation setting.
- Graduation from an accredited Occupational Therapy program.
- Strong clinical assessment, problem-solving, and communication skills.
- Ability to work independently and as part of a multidisciplinary healthcare team.
