Job Summary:
FULL-TIME (40
HRS) NOC SHIFT. Approximate pay range: $55.26 - $64.13 per hour. The Registered Nurse provides direct care in the Skilled- subacute unit. Communicates with physicians / Nurse Director / co-workers about changes in the patient s clinical condition, including results of diagnostic studies and symptomatology. Responds quickly and accurately to changes in condition or response to treatment.
- Consistently uses two patient identifiers when drawing lab work, administering medications, and performing procedures.
- Does not use unacceptable abbreviations in documentation; uses read back on all telephone orders.
- Consistently follows medication reconciliation policy.
- Ensures crash cart defibrillation function has been checked per protocol and after each code, appropriate items are immediately replaced and indicated by signature on crash cart checklist.
- Complies with the Infection Manual (OSHA blood borne pathogen standards, CDC Guidelines) and demonstrates appropriate use of personal protective equipment and handwashing techniques.
- Provides thorough, detailed, efficient shift-change reports to ensure consistency in the plan of care.
- Assists physicians with special tests and procedures in the unit; uses time out on all patients for any bedside procedures.
- Prevents injury to self and patients by applying the principles of body mechanics; maintains a safe environment with safety awareness and readiness (e.g., suction and oxygen available when needed).
- Performs pain assessment and reassessment with the time frame after administering pain medications and upon discharge or transfer of patients.
- Completes documentation of medications given to the patient on the MAR and narcotic sheet / Pyxis.
- Recognizes signs of abuse and reports appropriately to Adult Protective Services.
- Initiates discharge and patient/family education planning with proper documentation in the patient record (Medi-Tech).
- Establishes priorities of patient care based on essential needs and available resources (time, equipment, personnel, and supplies).
- Formulates and maintains a goal-directed plan of care that is prioritized and based on patient outcomes.
- Performs other duties as assigned or required.

Required Qualifications- Must have a current and valid license as a Registered Nurse issued by the State of California.
- BCLS upon hire and maintain current.
- Previous six months experience in Acute Care hospital experience or Long Term Care.
