Position: Health Services Director (LVN) - Lead Resident Care & Meds

Health Service Director – LVN – Oakland, CA

The Health Services Director is a licensed nurse responsible for coordinating resident care and medication management under the medical direction and supervision of the residents’ attending physicians, ensuring residents remain as independent as possible.

Responsibilities
  • Communicate with community healthcare professionals including physicians, discharge planners, nurses, and state agencies about residents’ care needs.
  • Coordinate incidental medical, dental, vision, hearing, and podiatry care for residents.
  • Perform nursing duties per scope of the Nurse Practice Act, administering medications and treatments as ordered by a physician.
  • Oversee and provide training for Medication Technicians.
  • Administer resident pre‑admission assessment prior to move‑in and perform routine and as‑needed reassessments per company policy and state regulations.
  • Interview potential residents and families and obtain medical records and other requirements per Title 22 for admission.
  • Conduct staff interviews and observe residents to alert Executive Director and others of changes in physical or emotional health and provide information on staff ability to meet needs.
Qualifications
  • Licensed Vocational Nurse (LVN) or Registered Nurse (RN) with valid California license in good standing.
  • Prefer 2 years of experience in a Residential Care Facility for the Elderly (RCFE).
  • Prefer 1 year of experience supervising employees.
  • Ability to obtain/maintain valid RCFE license.
  • Effective teaching/training skills for staff.
  • Proficient with personal computer and MS Office (Word, Excel).
  • Must pass background clearance and health screenings before employment.
  • Must be highly motivated and able to work independently.
  • Valid first aid and CPR certification.
  • Must pass criminal background check, health screening, physical and TB tests.
  • Compliance with COVID‑19 vaccination, screening, and PPE requirements per regulations.
