Position Overview
Licensed Vocational Nurse (LVN) – Hospice Sequoia Hospice of the East Bay – Contra Costa County, CA.
Pay Range: $35.00 – $42.00 per hour. Job Type: Full Time, Part Time, Per Diem, On Call.

Key Responsibilities- Provide direct patient care under the supervision of a Registered Nurse, following the established plan of care.
- Respond to on‑call needs, including symptom management, medication administration, and emotional support.
- Accurately document all patient interactions and care provided during the shift.
- Educate patients and caregivers on care techniques and symptom management.
- Communicate effectively with the interdisciplinary team regarding patient status and needs.
- Ensure appropriate use of equipment and supplies.
- Evaluate patient response to treatments and medications.
- Participate in quality improvement activities and professional development.
- Perform other duties as assigned by the supervising RN.
- Graduate of an accredited vocational nursing program.
- Active LVN license in the State of California.
- Minimum 3 years of nursing experience; hospice or community health experience preferred.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and auto insurance.
- Strong verbal and written communication skills.
- Ability to work independently during overnight hours.
- Understanding of hospice philosophy and the needs of the terminally ill.
