Registered Nurse - Skilled Nursing Facility 29 Job Summary

The Registered Nurse provides direct care in the Skilled‑subacute unit. Communicates with physicians, Nurse Director, and co‑workers as appropriate about changes in the patient's clinical condition, including results of diagnostic studies and symptomatology. The RN is able to respond quickly and accurately to changes in condition or response to treatment.

Responsibilities
  • Use two patient identifiers when drawing lab, administering medications, and performing procedures.
  • Avoid unacceptable abbreviations in documentation; use read‑back on all telephone orders.
  • Consistently follow medication reconciliation policy.
  • Ensure Crash Cart and defibrillation function are checked per protocol and after each code; replace items immediately and sign the crash cart checklist.
  • Comply with the Infection Manual (OSHA blood borne pathogen standards, CDC Guidelines) and use appropriate PPE (gloves, gowns, masks, goggles) and handwashing techniques.
  • Provide a thorough, detailed shift report to ensure consistency in the plan of care.
  • Assist physicians with special tests and procedures; use time‑out for all bedside procedures.
  • Avoid and prevent injury to self and patients by applying body‑mechanics principles; maintain a safe environment with suction and oxygen readily available.
  • Consistently perform pain assessment and reassessment after pain medication and upon discharge or transfer.
  • Complete medication documentation on the MAR and narcotic sheet/Pyxis.
  • Recognize signs of abuse and report appropriately to Adult Protective Services.
  • Initiate discharge and patient/family education planning with proper documentation in Medi‑Tech.
  • Prioritize patient care based on essential needs and available resources.
  • Formulate and update a goal‑directed plan of care based on patient outcomes.
  • Perform other duties as assigned or required.
  • Qualifications
  • Current and valid Registered Nurse license issued by the State of California.
  • Basic Cardiac Life Support (BCLS) upon hire and must maintain certification.
  • At least six months of recent experience in acute‑care hospital or long‑term care.
  • High School Diploma/ GED or higher.
