Job Summary

FULL-TIME (1.0) PM SHIFT APPROXIMATE PAY RANGE: $55.26 - $64.13. The Registered Nurse provides direct care in the Skilled‑subacute unit. Communicates with physicians, the Nurse Director, and co‑workers about changes in the patient’s clinical condition, including results of diagnostic studies and symptomatology. Responds quickly and accurately to changes in condition or response to treatment.

DUTIES AND RESPONSIBILITIES

Consistently uses two patient identifiers when drawing labs, administering medications, and performing procedures.

Does not use unacceptable abbreviations in documentation; reads back all telephone orders.

Consistently follows medication reconciliation policy.

Ensures crash cart and defibrillation function are checked per protocol and after each code; immediately replaces and signs items on the crash cart checklist.

Complies with the Infection Manual (OSHA blood‑borne pathogen standards, CDC guidelines) and demonstrates appropriate use of PPE (gloves, gowns, masks, goggles) and hand‑washing techniques.

Provides a thorough, detailed shift‑change report to ensure consistency in the plan of care.

Assists physicians with special tests and procedures in the unit; uses a timeout on all patients for any bedside procedures.

Avoids and prevents injury to self and patients by applying body‑mechanics principles. Maintains a safe environment, ensuring suction and oxygen are immediately available when needed.

Consistently assesses and reassesses pain with appropriate time frames after administering pain medication and upon discharge or transfer.

Completes documentation of medications given on MAR and narcotic sheet/Pyxis.

Recognizes signs of abuse and reports appropriately to Adult Protective Services.

Initiates discharge and patient/family education planning with proper documentation in the patient record in Medi‑Tech.

Prioritizes patient care based on essential needs and available resources of time, equipment, personnel, and supplies.

Formulates, maintains, and updates a goal‑directed plan of care that is prioritized and based on patient outcomes.

Performs other duties as assigned or required.

Required Qualifications

Must have a current and valid license as a Registered Nurse issued by the State of California.

BCLS upon hire and maintain current.

Previous six months of experience in an acute‑care hospital or long‑term care setting.
