Job Description SummaryAn Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or in an alternate infusion suite, ensuring patient safety and compliance under a physician’s plan of care. The role operates under structured supervision of a designated supervisor.
Job Responsibilities- Initiate, develop, and implement intermediate nursing plan of care treatments, evaluating patient progress toward goals.
- Organize and participate in the provision of direct patient care, performing treatments, administering medications, and educating patients and families/caregivers.
- Modify plan of treatment in response to changing patient status or physician orders.
- Assess patient needs and physical status at each skilled visit; re‑evaluate needs through physical reassessment and supplemental physician orders.
- Obtain and clarify physician orders for plan of treatment revisions, informing physicians promptly of significant changes and providing written summaries.
- Provide training and mentorship to Infusion Nurse I’s as needed.
- Teach patients and families using a patient‑centered approach for independence with prescribed therapy.
- Communicate effectively and timely with the clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care.
- Prepare real‑time clinical documentation during visits and submit to the nursing department to comply with billing timelines.
- Coordinate discharge planning and prepare discharge summaries with patient instructions, progress, and goals.
- Demonstrate compliance with agency operations, policies, procedures, professional standards, and regulatory requirements.
- Maximize work efficiency through the use of computers and automation technologies for order validation and communication.
- Respond promptly and appropriately to patient requests; initiate emergency procedures when necessary.
- Accept accountability for own practice through ethical and professional conduct.
- Adhere to legal and ethical guidelines for patient confidentiality, including HIPAA regulations.
- Explain Option Care’s scope of services and related financial obligations to patients.
- Complete required training modules, in‑services, and continuing education to maintain competency and professional licensure.
- Provide oversight and coordination of paraprofessionals in the home setting and may act as Supervisor when the Supervisor or Nurse Manager is absent, in accordance with regulations.
- Participate in multidisciplinary team conferences and mentor other nurses for orientation and onboarding.
- Participate in nursing department on‑call responsibilities.
- Perform other related duties as directed by supervisor.
- Insert PICC/Midlines per physician order.
- Troubleshoot or clear obstructed central line catheters.
- Monitor PICC/Midline and educate on infection prevention and routine care.

Basic Education And/or Experience- Active and unrestricted RN license in the state of practice.
- Minimum of 1 year of nursing experience.
- Current CPR certification required.
- Demonstrated competency in patient care standards and infusion skill sets applicable to agency programs and service needs; advanced certification and training as applicable.
- Required licensure to operate a motor vehicle in the state of practice with proof of liability insurance.
- Basic knowledge of computer operating systems and software applications; ability to evaluate annual appraisal and set professional growth goals with nurse manager.
- Ability to effectively use nursing technology tools to communicate and document care provided.
