Part-time Registered Nurse Case Manager
Territory:
Sacramento County
Schedule:
Every Saturday and Sunday Full Days. Optionally Saturday/Sunday plus 3 weekdays.
- Conduct Start of Care (SOC) Admissions – OASIS
- Conduct follow‑ups and evaluation visits as needed in designated territory
- Make home visits to patients in designated territory
- Perform assigned duties, including medication reconciliation, wound care, treatments, and procedures
- Monitor clients’ conditions and report changes to the Clinical team
- Collaborate with physicians and agency staff to deliver the best care possible
- Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided
- Concierge support for Case Managers – dedicated resources assist with time‑consuming administrative tasks
- Flexible work schedules – create your own schedule and hours
- Targeted territories – typically patient visits are within a 5–15 mile radius
- Autonomy – no clocking in/out

- Current and unrestricted CA RN license
- Current BLS/CPR certification
- Valid CA driver’s license
- Experience conducting assessments and Start of Care (SOC) – OASIS is highly desired
- High level of clinical knowledge in home health care is highly preferred
- Experience with high‑acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals are highly desired
- Previous experience with IV medication infusions is highly preferred
- Proficiency initiating intravenous (IV) therapy is a plus
