Part-time Registered Nurse Case Manager

Schedule:

Every Saturday and Sunday Full Days. Optionally Saturday/Sunday plus 3 weekdays.

• Conduct Start of Care (SOC) Admissions – OASIS
• Conduct follow‑ups and evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including medication reconciliation, wound care, treatments, and procedures
• Monitor clients’ conditions and report changes to the Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided

• Concierge support for Case Managers – dedicated resources assist with time‑consuming administrative tasks
• Flexible work schedules – create your own schedule and hours

• Current and unrestricted CA RN license
• Current BLS/CPR certification
• Valid CA driver’s license
• Experience conducting assessments and Start of Care (SOC) – OASIS is highly desired
• High level of clinical knowledge in home health care is highly preferred
• Experience with high‑acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals are highly desired
• Previous experience with IV medication infusions is highly preferred
• Proficiency initiating intravenous (IV) therapy is a plus
