Upward Health is seeking a Nurse Practitioner (NP) to deliver high-quality, patient-centered primary care primarily via our virtual platform with some in-person support for transitions of care. The NP will work closely with a multidisciplinary team and provide leadership in daily operations.
You will conduct primary care and urgent visits, oversee annual wellness exams, and ensure proper screenings and quality measures. Multi-state licensure and willingness to travel are required.
